50 yr old female with c/o of dov in both eyes

50 year old female with c/o of dov in both eye since 2 months .

History of present illness

Apparently asymptomatic 2 months back then developed dov in both eyes which was insidious in onset , gradually progressive .

History of past illness 

HTN + 
DM - 
Asthma - 

Personal history 

Married 
Occupation - Farmer 
Appetite - normal
Diet - mixed 
Bowel - regular
Micturition - normal 
No know allergies
No habits or addictions 

Family history 

Not significant 

Physical examination

No pallor, icterus, cyanosis, clubbing of fingers ,lymphadenopathy, malnutrition .
No oedma of feet .
Temperature - 98.6 C /F
Pulse rate - 80/min
Respiration rate - 17/min
BP - 130/90 mmHg
Spo2 - 99%


Systemic examination

Abdominal examination

Shape - scaphoid 
No tenderness
No palpable mass
Normal hernial orifices
No free fluid 
Ni bruits
Liver and spleen are not palpable 
Bowel sound - yes

Respiratory system

No dyspnoea , wheeze
Position of trachea - central
Breath sounds - vesicular 

CNS - NAD

CVS 

Ni thrills, cardiac murmurs
Cardiac sounds - s1s2 + 
                 RIGHT EYE 
                   LEFT EYE


Provisional diagnosis 

Both eye IMSC ( Immature senile cataract ) 

Investigation 

Serology , RBS 

Medication
