 50 year old female came to the hospital with chief complaint of dengue NS1 antigen positive



Past illness

No history of surgery , asthma, TB ,CAD, epilepsy , Diabetes mellitus 


Present illness

No cold, cough , Shortness of breath 
No B/I Pedal edema , no headache , no giddiness

Personal history 

Normal appetite
 Bowel movement - normal 
Bladder movement - normal 
No addictions 
Sleep - adequate 

Family history 

No diabetes , Hypertension, heart disease , stroke , cancer , TB , asthma 

General examination 

Patient is conscious ,
Coherent ,
Co- operative 

There are no signs of icterus , clubbing , pallor , cyanosis , Lymphadenopathy 

Vitals 

Temperature - 98.8 C/F 
Pulse rate - 80bpm 
Respiratory rate - 18/min 
Bp - 110/70
Spo2 - 96 

Systemic examination 

CVS 

No thrills 
Cardiac sounds - S1 ,S2  heard 

Respiratory system 

Position of trachea - Central 
Breath sounds - vesicular 
Adventitious sounds - no 

Abdomen 

Shape of abdomen - scaphoid 
No tenderness 
Palpable mass no 
Hernias orifices - normal 
Free fluid no 
Bruits no 
Liver - not palpable 
Spleen - not palpable
Bowel sound - yes 

CNS 

Patient is conscious 
Speech - normal 
No sign of meningitis 
Motor and sensory nervous system - normal 
Craial nerves - intact
