A 28 year old female with pain in the right hypochondrium and umbilical region .

A 28 year old female came to hospital with pain in the right hypochondrium and umbilical region since 7 months .

History of present illness
Patient was apparently asymptomatic a 7months back and later developed pain in right hypochondrium which is of cholelithiasis the pain is not radiating the pain is relieved by taking medication .
No history of fever, vomiting, constipation, burning micturition.
History of past illness 
No history of diabetes mellitus , hypertension, asthma, TB. 
Personal history 
Occupation - house wife
Appetite -normal 
Diet -mixed
Bowels -irregular
Micturition-normal
No know allergies, addictions
No know family history 
Physical examination 
Vitals
Temperature - 98°f 
Pulse rate - 85beats / min
Bp- 120/80 mm Hg 
Spo2 -99%
 Systematic examination 

Cvs 
No thrills, cardiac murmurs 
Cardiac sounds s1s2 + 

Respiratory system 
No dyspnoea ,wheeze 
Position of trachea central 
Breath sounds vesicular 

Abdomen 
Shape - scaphoid 
No tenderness , palpable mass , bruits, free fluid 
Normal hernial orifices 
Spleen and liver not palpable 

CNS 
Level of consciousness - conscious 
Speech - normal

Cholelithiasis
